Friday Night Lights Hydration: Why Football Players Are the Highest-Risk Athletes for Heat Illness

Football players are the highest-risk athletes for exertional heat illness in American high school sports—not by a little, but by an order of magnitude. A national surveillance study of U.S. high school athletes found football’s heat-illness rate was 11.4 times that of all other sports combined, with most cases occurring in August and nearly a third of practice cases hitting more than two hours into the session. The reasons are structural: a helmet and pads trap heat, linemen carry the most body mass, and the season starts in the hottest weeks of the year. Hydration cannot fix all of that, but it is the one variable every player controls.

What the Numbers Actually Say

The study, published in the American Journal of Preventive Medicine by Kerr, Casa, Marshall, and Comstock, analyzed six seasons of National High School Sports-Related Injury Surveillance System data (2005–2011). Across all sports, exertional heat illness occurred at 1.20 per 100,000 athlete-exposures. In football it was 4.42 per 100,000. Six in ten cases (60.3%) happened in August, and about a third (33.6%) began when no medical professional was on site.

The 32% figure—heat illnesses that started more than two hours into practice—is the one parents and players should sit with. Heat illness is rarely a first-fifteen-minutes problem. It is a cumulative one: sweat losses that were never replaced during the first hour catch up in the second.

Why Football Specifically

Equipment. A helmet and shoulder pads cover the head and torso, where a large share of heat leaves the body. Sweat under pads does not evaporate, so it does not cool. The NATA position statement on exertional heat illnesses (Casa et al., Journal of Athletic Training, 2015) singles out equipment as a risk factor and is the basis for the phased heat-acclimatization rules many state associations now use—helmets only for the first days, shells before full pads.

Body mass. Linemen produce more heat per drill and dissipate it less efficiently. The National Federation of State High School Associations notes that linemen are the group most often affected by heat illness and the reason acclimatization periods exist at all.

The calendar. Preseason camps start in the hottest weeks, and the September schedule—Thursday walk-throughs, Friday night games under lights that do not cool the field much, Saturday film—keeps players in a weekly cycle of sweating and only partially rehydrating.

The Hydration Rules That Matter

The NATA statement’s core hydration guidance is to keep fluid losses under two percent of body mass during activity, and the practical tool for that is the scale: weigh in before practice and after, in the same clothing. Every pound lost is roughly 16 ounces of fluid to replace before the next session. A player who weighs 180 pounds and finishes practice at 176 has lost more than two percent and should not be starting the next practice at that weight.

Between weigh-ins, pacing beats volume. NIOSH’s recommendation for moderate work in the heat—about a cup of water every fifteen to twenty minutes—is a reasonable frame for a practice with regular water breaks, and it is far more effective than drinking a quart at the whistle. For sessions that run past two hours with heavy sweating, the same guidance suggests a drink with electrolytes, since sodium losses under pads are substantial.

Off the field, the missing piece is usually the day before. A player who arrives at Thursday practice already behind from a dehydrated Wednesday night has no margin. A bottle that goes to every class and gets refilled at lunch is the simplest fix for that.

Recognizing the Emergency

Exertional heat stroke is a medical emergency, and the single most important update in the 2015 NATA statement is “cool first, transport second”: a suspected heat-stroke patient should be immersed in cold water on site until body temperature drops, before being moved to a hospital. Warning signs include confusion, irritability, stumbling, collapse, or a player who stops making sense. If a teammate looks like that, the response is to get an adult and get the player into cold water and shade—not to hand him a bottle and wait.

The Player’s Bottle
