Does Creatine Dehydrate You? What the Research Actually Says About Water and Creatine

No—creatine does not dehydrate you, and it does not cause muscle cramps. The International Society of Sports Nutrition’s position stand on creatine reviewed the research and concluded that claims linking creatine to dehydration and cramping are not supported by the evidence; several studies found supplementation either had no effect on those outcomes or was associated with fewer cramps and heat-related problems. What creatine does do is pull a modest amount of water into muscle cells, which is why the “drink more water on creatine” advice exists. The honest version: keep your normal fluid habits, and do not expect creatine to change them much.

Where the Myth Came From

Creatine is osmotically active. When muscle cells store more of it, they hold more water inside the cell, and body weight typically rises by a couple of pounds during the first week of loading. Somebody looked at that intracellular water and reasoned that it must be coming from somewhere—the blood, the rest of the body—leaving the athlete “dehydrated” and cramp-prone. It was a plausible guess in the late 1990s. It just has not held up.

The ISSN position stand (Kreider et al., Journal of the International Society of Sports Nutrition, 2017) reports that creatine supplementation does not increase the incidence of musculoskeletal injury, dehydration, muscle cramping, or gastrointestinal distress beyond what is seen with placebo. It cites collegiate football players taking 20–25 grams a day for twelve weeks through spring practice with no adverse effects, and it notes that some studies found lower rates of cramping and heat illness in supplemented athletes. A 2021 follow-up in the same journal (Antonio et al., “Common questions and misconceptions about creatine supplementation”) addressed the dehydration question directly and reached the same answer.

What Creatine Actually Does to Your Water

Roughly two-thirds of the water in your body is inside cells. Creatine shifts a little more of it there. That is a redistribution, not a loss, and the body is very good at defending its total water and blood volume through thirst and the kidneys. The practical effect most people notice is the scale: a one- to two-kilogram bump during loading, most of it water in muscle, which stabilizes afterward.

Because that water is intracellular and stays put, there is no evidence it increases what you sweat out or urinate out. Your daily water need—the National Academies’ adequate intake is about 3.7 liters of total water a day for men and 2.7 for women—does not change on creatine in any documented way. What changes is your training: if creatine lets you do more work in the gym, you sweat more, and that is what should drive extra drinking.

So Should You Drink More?

Do not drink less. The one real risk in the creatine-and-water story is people who take it and then, for whatever reason, under-drink during hard training in the heat. Creatine did not cause that; the under-drinking did. The ISSN’s advice is to maintain adequate hydration, the same advice it gives athletes who take nothing.

Time it with a real drink. Creatine monohydrate dissolves better in warm liquid and absorbs fine with water, juice, or a shake. A common and reasonable pattern is 3–5 grams a day mixed into the first 8–16 ounces you drink after a workout—which turns a supplement into a hydration cue.

Watch training, not the tub. If you are doing more volume because creatine works, weigh yourself before and after sessions. Each pound lost is roughly 16 ounces to replace. That is the number that matters, and it has nothing to do with the powder.

Who Should Ask First

Creatine is one of the most-studied supplements in sports nutrition and the ISSN calls it safe for healthy people at recommended doses. People with kidney disease, anyone on medication that affects the kidneys, and anyone under 18 should talk to a clinician before starting. The same goes for anyone already on a fluid restriction for a medical reason, since a supplement that draws water into muscle is worth a conversation in that case.
